On 27 October 2015 the NSW Cabinet Expenditure Review Committee requested David Tune AO PSM conduct an inquiry into out-of-home care in NSW.

The final report found that the NSW out-of-home care was ineffective and unsustainable. The number of children in out-of-home care had doubled over the past ten years in NSW. The system had failed to improve long term outcomes for children, particularly Aboriginal children. An investment approach should be introduced, similar to that established in New Zealand. A new NSW Family Investment Commission should be established to coordinate and lead these reforms.
